Adoption Applications

Required: You must reside within 50 miles of Chicagoland area.
PLEASE READ BEFORE FILLING OUT APPLICATION!

What is needed:

  1.   At least 21 years of age, person adopting the dog MUST fill out the application
  2.   Valid Id with a current address
  3.   If Renting, document to prove permission to own an animal
  4.   All adults in household must meet and approve in ONE meeting session, no multiple meetings will be set up.
  5.   Talk with Adoption Counselor/Foster Parent
  6.   Follow up with either training and/or Neutering/Spaying as directed.
  7.   Home Visit must be conducted for safety check
  8.   Certain instances will require a background check
  9.   Complete an Adoption Application in its entirety
  10.   Meet with Team Member to go over Adoption Contract
  11.   In those cases where there are more than one adoption application on a dog, please note we will NOT be contacting you with regret letters.  If you do not hear from a member, another family was chosen for the dog.
  12.  Adoption Fees are CASH ONLY.  No Checks.
  13. FEES:  Include all vaccinations to date of adoption.  Spay/Neuter and Microchip.
  14. Cost to Adopt is determined by the age of the dog. 
  15. We do NOT allow dogs to go to homes who will be utilizing an electric fence.  Proper Positive Reinforcement Training is recommended instead. 

As of January 1st, 2013:

Puppy to 2 Years of age: $320.00 

2 Years to 7 Years of age:  $295.00

7 Years and older $145.00

  1. All Adoptions will take place at either the Aurora or Mt. Prospect Petsmart stores on a Saturday.
  2. You MUST be willing to travel to meet the dog at the Fosters request.    We do not “deliver” or “ship” any of our dogs.
  3. Puppy adoptions (those dogs under the age of 6 months) will need to have 1 adult home to train properly.  NO EXEPTIONS.
  4. Puppies under the age of 2 years of age must attend a puppy train class.  Speak with Adoption Counseler for details.
